Doctor: Good morning, come on in. What brings you in today?
Patient: Hi doctor. Um, so I've had this like, nagging cough for about 2 weeks now, and lately I've been getting, you know short of breath, climbing stairs.
Doctor: Okay 2 weeks of cough with some exertional shortness of breath, any fever chills or night sweats?
Patient: No fever that I've noticed, but um, I have been more tired than usual, like more than normal.
Doctor: Any chest pain, or coughing up blood or discolored sputum?
Patient: No blood. Sometimes there's, like a little yellowish mucus, in the morning, you know, when I wake up.
Doctor: Do you smoke, or have you been around anyone, who's been sick recently?
Patient: I quit smoking about 5 years ago, but um, my coworker had a bad cold last month, so like, maybe that's it.
Doctor: Got it, let's listen to your lungs, take a deep breath for me, again.
Doctor: Okay I'm hearing some mild, wheezing in the lower right lobe. I'd like to start you, on a 5 day course of azithromycin, 250mg, and an albuterol inhaler, for the wheezing. Let's also get a chest x-ray, to rule out pneumonia.
Patient: Okay um, that sounds good.
Doctor: Given the wheezing hasn't cleared up on its own, I also want to refer you, to pulmonology for a follow-up evaluation, and possible pulmonary function testing.
Patient: Okay yeah that makes sense, I guess um, better safe than sorry.
Doctor: I'll get that referral sent over today. In the meantime, I want to see you back here in 1 week, to check how you're responding to the antibiotics, sooner if the shortness of breath gets worse, or you develop a fever.
Patient: Okay um, so next week works, yeah.
Doctor: Great, let's also go ahead, and schedule a follow-up appointment, for 3 weeks out, after the pulmonology visit, so we can go over their notes together. Does a Tuesday afternoon work for you?
Patient: Yeah um, Tuesday afternoon should be fine, like any time after 2, is good for me.
Doctor: Perfect, we'll pencil you in. Any questions for me?
